Interventricular septal hematoma detected by transesophageal echocardiography after congenital heart surgery in an infant: a case report

Abstract Background Interventricular septal hematoma is an extremely rare complication following congenital heart surgery. During cardiac surgery, interventricular hematomas can be detected only by intraoperative transesophageal echocardiography. Here, we report interesting case of that was accidentally found in infant ventricular defect (VSD) closure. Case presentation Transesophageal echocardiography images were acquired from a 1-month-old boy after surgical repair large (6.5 mm) perimembranous outlet VSD with flattening. Surgical correction performed auto-pericardium and 7–0 Prolene sutures. The patient successfully weaned cardiopulmonary bypass, showed no leakage good function. However, approximately 30 min later, two anechoic masses within the septum, which suspected to hematomas; larger mass measured 1.51 $$\times $$ × 1.48 cm. swollen septum decreased contractility compressed both right left ventricles. there change size or significant hemodynamic instability for observation. Therefore, expecting spontaneous resolution hematomas, not explored, removed bypass. On postoperative day 4, follow-up transthoracic revealed thrombi filling hematomas. discharged on 15 followed up regular echocardiographic evaluations. Conclusions We describe unique manipulation injury perforating artery may contribute development hematoma. Moreover, conservative treatment serial evaluation generally show gradual hemodynamically stable patients. Pediatric anesthesiologists should aware this repair.
